I'd like to order two sticks but the rebate form says 1 rebate per household, per address, per person, per receipt. What if I make two separate purchases using two unique mailing addresses (i.e. home and work) do you think that they will honor both rebates?

Thats what I did. It has worked in the past. Rebate form says one per household, address, and receipt, I would stick a wife or gf's name on the second one to be on the safe side.

Got my ram today. Anyone got a copy of the rebate form, I thought I printed a copy, but I cant find it. 😱
 
The rebate states... Mail this completed form with the original rebate or sales receipt
and the PLU/rebate sticker cut from the back of the module plastic packaging to:
Mine didn't come with such a sticker just a bar code sticker. I wonder if they will accept it?

Same boat. At the bottom of the rebate form it states again:

*Only Patriot modules with the PLU# ****** on the sticker is valid for this rebate

I've cut the labels form both sides of the package and including them with the rebate. I hope this doesnt have to get ugly. $45.78 is too much for a 256MB stick of generic PC2700.

If you haven't received your rebate yet, you better call them. My emails came back with errors due to their's being "over limit". After leaving a terse message on their voice mail, I got an email back in about 30 minutes confirming that I'd get my check in the next two weeks.

Wow, i forgot about this rebate. I guess I'm going to have to give them a call.
