Depends on your locality, to a certain extent. Many bond issue fights over it locally. It's in the budget now here for emergency services.

Here, we pay an "emergency services" bond on our property taxes and are billed monthly in our water/sewer bill for ambulance services, but should you actually need a ride in an ambulance...it's gonna start at about $2000 for a trip to the closest hospital...about 25 miles away.

Yep, the first responders are paid through taxes here. If you need transport, a second, private ambulance hauls you in at $2k for the ride. Folks have learned to let the first responders stabilize them and then refuse transport and find their own way to the hospital.
