I think it's just referencing pretty much anywhere in the country where there is a larger group of conservatives and where the homophobic GQP base is more prevalent, where LGBTQ youth feel shitty and unable to be themselves, and living space (including closets) is on average more expansive, so quite a big area of the country. People often shit on NYC as it's so expensive to live and you get small spaces to live in, so it's a pretty valid point and addressing the latter point, acknowledging we do indeed have smaller living spaces, but X. It didn't really specify a comparison to other cities or not, just general areas. It's an easy guess as to what areas they are talking about.

I think it makes perfect sense.
