If a flight involves an overnight night stay in a foreign country (in my case 21 hours in Shanghai between flights), do I have to stay at the airport or can I leave to sleep in a hotel and do touristy things for a day?

I'm pretty sure you get a 3 day temp visa if travelling to a 3rd country while in transit, there are a couple of hotels close/at the airport, i'd book one of those and also see if there are any day trips that you can take from there. I might be headed through that airport later this year as well, China Eastern has had some amazing deals lately.
In order to facilitate the international visitors' short stay in China, 72-Hour Visa Free Transit policy is carried out in more and more cities. At present, eighteen cities adopt this policy. This policy allows transit passengers with passport of 51 countries to stay for up to 72 hours without a visa on direct transit.
The "23 hour Layover" is something many travelers intentionally try to do, (depending on the country, visa restrictions, etc.) in order to extend vacation time for little or no extra airfare.
