There's always the Tampa Pitcher Show in Tampa. It's more of a Restaurant/Theatre, however. For instance, there are round tables instead of just seats.

I think the Tim Hortons/Wendy's combos are showing up in the US as well (Michigan and northern NY, I think). The coffee there is pathetic IMHO, but there are many people who love it.
 
Carousel Grand in Greensboro, NC. There's a bar close to the smaller auditoriums that generally show the independent films. I always bring my own though, never had a problem.
