Questions.

1. Do I need to leave ATT on for the settings to work?
2. How do I enable Triple Buffering for D3D? I see the option but it's affiliated with profiles, which I have no idea how to work.

In general you don't but it depends which settings.

2. How do I enable Triple Buffering for D3D?
You can't force it in Direct3D due to the API restrictions. You'll have to enable it from the game.

All this whining from some people about how Triple Buffering/D3D didn't work on NVIDIA drivers and it doesn't work with ATI ones either? Or was it OpenGL that the NVIDIA ones didn't work with?

Yup, OpenGL. According to the ATi driver engineers forcing triple buffering in Direct3D is not possible due to the API.
