I think it has something to do with the amount of ADAM you ingest, our hero uses some ADAM and gets tons of powers (that can clearly be seen deforming his hands) yet if you do the "good" ending he lives a normal life and dies of old age, a "deformed" spliced up being wouldn't have lived a normal life in the surface.

As far as Fontaine goes... he could have been that "hulked up silver surfer" thingy from the very beggining, what makes you think Fontaine didn't splice till the end?

As far as Ryan goes... well, he simply could have NOT used ADAM like you suggest, knowing of its destructive properties or ingesting very little (notice he doesn't have powers, maybe he used as much ADAM as to fix a broken bone or a crooked nose or something).

We never see Tenenbaum up front, for all we know she could be spliced, being deformed doesn't necessarily mean you act like the splicers that attack you, Sander Cohen is spliced up yet he is "normal" and doesn't really attack you after you complete his piece. Also remember that even if Tenenbaum is spliced up to hell she still has quick access to ADAM so she wouldn't have the "delirium tremens" symptoms the other addicted splicers have.

And I don't remember that scientist but she might have been spliced as well, she just acted normal, simply because she had access to ADAM or wasn't as much of an addict as to go crazy like the other splicers.

To my knowledge, EVERYONE in Rapture was spliced and things were going "normal" until Fontaine started a revolution on New Year's Eve, the society collapsed and the splicers, not being able to get more ADAM went mad. If you notice, splicers are not zombies and when left alone act relatively normal, you even see a couple of them dancing on Sander Cohen's apartment, completely non-aggressive until provoked.

Just a little pointer that spliced up=/=insanity. As far as I know Ryan is the only person who shows strong evidence of NOT being spliced up at all.

Hopefully that made sense.

Pretty sure he mentions it in one of the very last tape logs you stumble upon. Might have to check when I get home tonight.

Yeah but he kind of makes clear that the only reason he didn't splice is not to mix business with pleasure and even says that he doesn't know what he was thinking when he didn't spliced.

The point still stands, splicer=/=insanity. The only person in Rapture confirmed not to be an splicer is Andrew Ryan. Tenenbaum might not be a splicer because she knew from the very beggining she discovered the slugs that they could bring side effects, in any case, her acting normal is definitely not evidence to support she isn't spliced.
