Just like BFG10K said, it's an app to control your TV Out feature. You can even use a monitor and a TV at the same time, only the refresh rates are limited.

It's a great app for enabling overscan on Nvidia cards, when using TV-OUT. Any of you that own a Nvidia card have noticed that there are black bars down the side of the picture when running TV-out, this program eliminates that. It costs $10 to register, I think, however I've yet to work up the nerve to purchase it as I don't use TV-OUT that often.
It'd certainly help for those of you who use TV-OUT on a regular basis though, cause NVidia's drivers at this stage absolutely STINK for producing a good tv-out image.

I've evaluated TVTOOL and have mixed feelings about it.

I'm very happy with the composite video output of the Gainward PowerPack G3 Ti200 Golden Sample, which uses the Conexant CX25871-13 chip. I've watched videos in the 2.2k-2.6k bps streaming rate on my TV and they are clean and very clear.

The only problem I have with Expertool from Gainward is that you have to pass through several steps to switch the output.

With TVTOOL, you have the ability to do this on the fly using hot keys, but the video quality for me was unstable. Maybe it is that the software has a lot of settings, and maybe I missed something (I played with the settings for a couple of hours), or that I was using version 5.5.3 and not the new 6.01 beta (supposed to fix a lot of GF3 issues). I dunno.

I would like to switch the video, and control the CX25871-13 chip via a script, or C++ or C#, but I've yet to find a driver class for it.

That's my 2 cents worth.

I've watched over a hundred DVDs with TVtool and a MX-400.

No particular issues with PQ or stability.

With what specific card and what version of TVTOOL? Could you tell me what your settings are? I've heard nothing but good things about TVTOOL. I just can't get it (v5.5.3) to work for me and my GeForce3.

Nah, the Conexant CX25871 in my Gainward PowerPack Ti200 Golden Sample is listed. I'm not using the nVidia detonator drivers though, but rather the updated version of what came with the card. The manual states that overclocking is not possible with the nVidia drivers, so I stayed with the ones I have. That might be what is causing the issue.
