What was the sole on-ice fatality? I remember a girl in the stands getting killed not too many years ago (Columbus Blue Jackets game?) after being hit by a puck, but that's probably not considered on-ice.

BTW, about neck guards... someone in Sweden did die from this type of thing and now all their players are required to wear neck guards. In the NHL, I'm sure it will remain an optional thing. This happened 2 or 3 times in 20+ years. It's a freak accident. You can get hit by a bus walking down the street too...
