The only time they don't do extra point attempts is if the TD points clinch the game without the extra point(s) try.

What if they're only up by one? Theoretically the other team can block the EP and run it back for two points, can they not?

you cannot run back blocked or intercepted/fumbled extra point tries in the NFL (unlike the NCAA)
