(New ?) Quick easy code update needed

I don't have access to my software at the moment and was hoping someone could post the code real quick for a radio button html page.

I just need a simple YES NO setup and when YES is clicked (no submit button) a page pops up and goes to a website (ie: http://www.yes.com). If someone could post it, It would be much appreciated!

This should do somehting along the lines of what you are looking for. You must also give your form a name, in this case, 'myForm'.
