IBM Tivoli Netcool and other associated products. Works via SNMP mostly. We had a serious Solarwinds install but its been pushed out as not scalable to our infrastructure.
Observium is a pretty simple to use and useful SNMP-based monitoring software. It's open source. Combined with smokeping (to graph latency and other service response times), which Observium can integrate with, you can get a pretty good idea of how your network is performing.
Another vote for Orion NPM including the netflow & sla analyzers. Works for us with about 30 devices in our core and about 200 deployed. Can toss in another 30 windows boxes.
I use IPSWITCH Whats Up GOLD v13 i believe. Very nice product, you can have it up and running in pretty much an hr or so. I have it monitoring servers and switches.

We have a total of around 2500+ nodes deployed and over 8000+ interfaces monitored. We have the Orion IP SLA, Neftlow, NCM and APM modules. Overall it is a great product that has come as close as possible to what we can find that combines a easy to manager system that gives us a single window in Network and Server equipment. You can monitor everything from ESX clusters to Cisco 6500 switches with it. It isn't perfect and has some kinks about it but overall it works fairly well. What I also like about it is that SolarWinds keeps adding more features and more value to the current products.
