Needed: OLD TDK Rebate Form for purchases BEFORE 12/31

Sorry mods, but this is critical. I have lost my rebate form for the TDK VeloCD 12/10/32X for purchases made before 12/31.

Sorry again to clog up the thread, but it needs to be postmarked by 1/31 to get in, and time is now of the essence... thanks all.
