Well, if you change your mind, my girlfriend is an ASID certified interior designer. Space utilization is pretty much an every day part of her job.
You don't want an architect. They'll just give you fucked up plans that are missing dimensions, and won't square up if you manage to find enough to go around the building ;^)
He is opening a clinic, so he will need permits and meet all local town/city codes.
So he best bet is an architect, so whatever he does, is all within the law.
