just type out what you want... then press ctrl+t for free transform to rotate it and change it to the size you want, and on the toolbar on the top, you should see this distortion like button. With that, you can distort the text to any shape you want.

The pic in THE OP makes it seem like she just ripped a large one.

eits there are lots of free programs you can use to accomplish this, depending on the look you are going for...sumopaint, paint.net....

To be honest though, i think this picture would look cheesy as hell with text coming out of the french horn. its a good picture as is, maybe just add some simple text in a corner or something, IMHO.

You have the tool you need: MS Paint.
1) Open the image in paint.
2) Give yourself room to type text by either clicking and dragging the bottom right corner or by changing the width/height in the "attributes" section.
3) Use the text tool to type the message in the new white area. You can use any font you want.
4) Use the select tool to select one letter.
5) Move it to where you want it. If you want the letter with a white background make certain the opaque option is selected, if you want no background make certain the transparent option is selected.
6) Repeat #4 and #5 for a few minutes.
7) Resize back to the original size.
8) Save the file.
