Need help...Fake doctors note

I've always wondered about workplaces that require this. It seems like a HIPAA violation waiting to happen. The employer would have to accept a note with no details on it, such as: It was recommended that xxx not go to work today.

That's what the notes say. "blah blah was under my care on blah blah date. The employee is authorized to return to work on blah blah date."
