I was thinking something with a relaxed atmosphere. I'm not in the mood for those places where everything is dark and you feel bad if you adjust your chair the wrong way.
Any type of food will do

Price: $70-100 for food and drinks for 2 ppl
Im a fan of places that have entertainment along with dining. (eg. dancing/singing/band)

EDIT: Yes, it's for a date. Sorry, no pictures.

Steak - Gibson's, Ruth's Chris, Magnum's (their signature bone-in ribeye is the best steak I've ever had).
Prime Rib - J Alexander's. I've found Lawry's is hit or miss.
Mexican - Fiesta Mexicana
Tapas - Cafe Iberico
Chinese - Chinatown...lots of places

It would help if you gave a price range and type of food.

Omega, it's in Golf. Great atmosphere, it'll run you about 50-100, but the waiters are friendly, the food is awesome, and it's 24/7.

You definitely have to have the House Special prime rib at the Chicago Chop House. It's a hefty bone-in slice of prime rib that's cooked medium rare then seared like a steak to give it a outer crust. I always make a visit there when I'm in Chicago on business. There's generally a guy playing piano in the lower dining area. Great place to bring a girl.

Last time I was in Chicago, I took my girlfriend to Maggiano's (Italian). Pretty good food and it was ~$60 without any drinks (alcoholic beverages that is).

Surprised no one mentioned it, but the House of Blues is really good food for a little less than your target price range with good entertainment every time I've been there (only twice, so take it with a grain of salt :p). It might not be as formal as you're looking for though.
