Here is some that are on the top of my list:

Borgan, original Lunar II. Gravity Bomb anyone?

Grilgan, Lost odyssey. Come on?? Aoe spam on a second boss??

Vay, Sega cd all the stupidly hard boss.
 
Emerald Weapon in FFVII after I accidently sold the underwater materia. :-X That took some trickery to make it happen.
 
Ruby Weapon in FFVII. Emmy was easy with KOTR+Quadra Magic+mime
Zeromus was kinda tough.
Culex, a hidden boss in Super Mario RPG, was tough without the Lazy Shell

I didn't feel like getting the underwater materia so I fought him with the timelimit.

Took 2 or 3 tries, but I remember playing late one night(2 or 3 in the morning and giving it one last shot) started my last knights ofthe round attack with under a minute left so it was my last shotor I drowned, and he died. I was thrilled.
 
All RPG bosses depend on level and items. For example Grilgan whooped me up, but then I leveled twice and whooped him up. I will give you that he was the hardest boss so early in a game.
I don't remember any of the bosses in Vay being difficult.

I am going to say the ending guy in Lufia: GUARD DAOS.
He would confuse everyone in the group and you would just sit and watch your group die over and over.
 
Ruby weapon from FF7 hands down. Decided it wasn't worth it after the 2nd or 3rd try. Emerald wasn't as difficult.
 
Optional fight with Sephiroth in Kingdom Hearts 2. You are knocked down to one hit point in the first hit. That was the only thing I didn't complete in that game.

I just forced the lucky 7777 on a couple party members, but that was a trick in itself. then used KotR a few times. This prevented drowning because 7777 does more damage faster than KotR. It also allowed me to remove some Materia so that Aire Tam Storm wouldn't do as much damage.
 
omg, I almost forget about Guard Daos. The on;y want I beat him was using reflect on all my party member so I won't get confused. Btw.... I through you can get underwater materia again if you morph those ships thing from underwater factory.

Finding him was not intuitive. IIRC, you had to go to the floating island where you use the flying chocobo to play the treasure hunting mini game. On this island is a large rock that you can walk up to and it has a gem looking like thing on it. Searching that gem reveals Ozma. However, he is completely unbeatable unless you killed all of those rare beasts that are scattered throughout the world. Once you kill the last one, a message appears mid battle telling you that "you can now fight the orb guy" or something like that. This brings Ozma to a position during the fight so that you are not limited to only being able to use ranged attacks against him.

I can't remember what level I was but I beat him too. Sephiroth in KH2 was much more about reaction timing than it was power leveling. Although, most of the RPG end game optional bosses do rely heavily on leveling.

he was the one in the locked door in that village late in the game. You had to buy some crystal from a kid I think to be able to open the door and fight him. Once you got to the top levels, he was pretty easy.
 
The last boss in Super Mario RPG was the hardest boss I've ever had to face... I don't know why, that guy was just really tough!

The Sephiroth fights in KH1 and KH2 were pretty tough, but got easy once you knew the pattern and how to deal with him (I can beat the KH1 Sephiroth without using potions or elixirs - it's all about timing). The Super Mario RPG last boss was just so unpredictable... and most of his moves unstoppable.
 
Chrono Trigger - trying to beat Lavos at the first opportunity. There can't possibly be a harder boss than that. Your party is so puny and weak, there's no way you can win so early in the game on the first play through!
