My ideal new phone

I'm looking for a new phone, but the last two or three phones I've had always get little bits of dust and such covering the lens... I'd like pictures to reliably be free of unnecessary defects (some defects are expected, given it's a phone, not a camera)

Any ideas for a camera phone that:
(1) Covers the lens(slider?), or otherwise keeps it clean
(2) Is relatively easy(usb, perhaps) to get the pictures out of the phone
(3) Is fairly durable (frequently twisted and bumped in my pocket, infrequently dropped, but it happens)
(4) Is otherwise decent- I like signal the most, battery life second, bells&whistles third

Is there a phone that meets all or most of those criteria?

Thanks in advance for your comments/suggestions!

I heard from a friend that the enV has a lens cap... I can't find pictures of it, is it true? Also, what sort of experience has anyone had with it?

Sony Ericsson K790 might fit the bill. It has a card slot so it is easy to transfer to it. It has like a 5 MP camera with a slider in front of it to protect it. Overall, it is kind of a thick candy bar form factor, but seems pretty durable.
