The original Max Payne came w/ a free mouse pad. I've been using that ever since I got it.

Funny thing is, I went to a LAN party a week ago and 4 out of 10 of us were using it. =P

Same here, some pad my mother grabbed from work many years ago when my last one started peeling between layers.

IMO mouse pads haven't mattered since the arrival of laser mice. Used to be a big deal with the ball mice, but now anything will do. Personally I'd prefer the small amount of friction the old pads give compared to the super slick ones these days.
