Cats use smell more than eyesight to identify familiar items. It is kind of amusing when one of my cats comes back from the vet. The other will sniff her and hiss.

What a mother cat does with kittens isn't that predictable. We had one that freaked and started moving her kittens around constantly, eventually found all of them dead in various places around the house.
