First time I entered Dynamis in FFXI. It was a battle between some epic bosses and 64 online players. My PC was shit at the time, I thought it was gonna explode when I was trying to cast a spell in the middle of a crowd. Also the first time I saw so much deaths in that game.

Or when Dynamis Lord one shot the whole allaince with his oblivion smash.
 
Joining the Blackwood Company in Oblivion and then killing the monsters attacking the village. Later you find out that you were hallucinating due to the drug they gave you and that you had actually slaughtered all of the villagers.
 
damn, i'm pretty depressed i can't think of anything. i'm not big on single player story modes anymore, maybe that's why.
 
the beginning train ride in Half Life. I bought the game on a whim, with no understanding of what I was about to get into. Load the game up and I'm riding on a train able to see the sights and thinking "Holy Crap, what did I just get myself into?".

Ya, not a lot happens on that ride, but it set the mood perfectly and really made the HL World seem real.
 
Going into the Underground Labs in Stalker SoC. It was so creepy that later when I was re-exploring areas for missed items, I started gong back into one of the Labs, then turned back because I didn't want to deal with all that again. 😀
 
Some people mentioned FEAR -

I think that spot where you start to go down a ladder and Alma is standing right there when you turn around, and then Fettel is at the botom of the stairs is probably the biggest single pure shock I've gotten in a game.

That sounds so incredibly complicated I'm gonna have to call bullshit. I seriously doubt the devs would have gone to all that trouble for something that would end up breaking the game. Cuz if you do it then you could have her in your party at times when they story mentions she's been murdered by Sephiroth. Did the devs also have alternate cutscenes for everything at the end of the game?
 
In a slightly different vein: Eve Online, where player Haargoth Agamar disbanded the Band of Brothers alliance to the surprise of everyone. The entire player base was all WTF about that for weeks, but it was a perfectly valid game mechanic.

Yeah I don't even play Eve and I heard about that. Technically valid but shouldn't be IMO. That's like the CEO of Ford 'disbanding' the entire company with a mouse click.
 
The first WTF moment for me was the beginning of System Shock 2, the ambience in that game was just crazy. When you have oncoming zombies running at you, and you have a wrench in your hand....ah good times. And scary at 1 am in a pitch black bedroom. LOL
 
The final level of Spear of Destiny (sequel to iD's Return To Castle Wolfenstein), after ripping through level after level of genetically altered and enhanced Nazi soldiers, that died as expected, you end up in a dungeon level and you're being chased by a bunch of slow moving ghosts that are easy to get away from. After a while get cornered by a swarm of them, so shoot one and watch him disapper in a "poof"...only to slowly fade back in to existance! I don't know why, but I remember getting such a chill down my spine! They could not be killed! They were like real ghosts! 😱
 
Personally, I was pretty crushed when I pulled out the Master Sword and turned into Old Link. I went from all cute and cuddly to being some freaky teenager.
 
I am a little surprised nobody has mentioned Planescape Torment.

Rather than one moment in that game being shocking the entire plot line was shocking, as was the way the story made you have empathy for creatures that would normally just be fodder for you sword in most games.

If any of you have not played it - pick it up from bargain bin or whatever. It is a classic that should not be missed. Heck, suddenly have an urge to play it again 🙂

When the hag Raven(I think that was her name) told you were were made immortal and were trying to find a way to end your own mortality was a bit of a shocker. As was the final scene, grabbing your mace and walking into a conflict of demons battling knowing it would give you what you seek - your own mortal death. That struck me emotionally and I still have not forgotten it - best ending ever.
 
Everything about Planescape Torment was awesome. One of the best stories, characters, settings, etc. in any game.

I would love a sequel or something similar but it would never live up to the original
