Blood and Blood2 - they started it... havent seen anything more nasty at that time... or since...
had good story and gameplay and way gorey

The option of attacking and/or killing not only innocent bystanders, but also police officers. During one segment of the game, Postal Dude must try to exit a meat-packing plant while being attacked by police; it is very difficult to complete this mission without using deadly force against officers.

Al-Qaeda terrorists who carry out a suicide bombing of a church and a marching band (these characters all resemble Osama bin Laden and many characters in the game of Middle Eastern descent are shown to be connected in some way with the terrorists).

The ability to urinate on bystanders to make them vomit in disgust. As cops will eat food items dropped by the player, urinating on a food item that is subsequently eaten by a cop will make the cop regurgitate.

Anthrax-filled cow's heads as weapons which make the victims vomit blood.

The use of cats as silencers for shotguns and machine guns, involving pushing the barrel of the gun into the feline's rectum.

When Postal Dude wears the police outfit and brutalizes innocent people, the other police officers openly admit that they are corrupt. At several points in the game, police NPCs are seen attacking and killing civilians for no apparent reason.

Crack cocaine "health" pipes that can be smoked in order to boost Dude's health to 125, above the usual limit of 100, no matter what value of health the Dude previously had. True to real life, these items will cause the Dude to become addicted, and if another dose is not used while the Dude is complaining about his withdrawal symptoms, his health will eventually be damaged, after which there are no further ill effects (unless another pipe is smoked).

Decapitating people with shovels and kicking the heads about.

Playing "fetch" with dogs using the severed heads of killed NPCs.

Pouring petrol on people and setting them on fire with matches.

Stun gunning people until they cower on the ground and urinate themselves.

A scene that bears resemblance to the Waco Siege of 1993, with a cult group being surrounded by ATF agents at "the compound".

Jihad, the company that makes the goat milk in Monday's chores

Homophobia in the shape of an arcade game prop called "i love you Hunter", as well as a gay dance club location

Racial stereotypes, such as Habib, the owner of the Lucky Ganesh convenience store, who speaks with an stereotypical Indian accent similar of that of Apu from the Simpsons.
 
Gonna have to agree with postal two.
While it isn't "realistic gore" at all, it is extremely violent.
It was my favorite deathmatch game at lans for a couple of years, just because of all the laughs ...
 
The Soldier of Fortune games are pretty messy.

Does anyone remember Kingpin: Life of Crime? I loved that game. "Fvck you mother fvcker!"
 
The most gruesome game I own/play for PC would have to be the Jedi Knight games with realistic lightsaber cheats activated (where limbs come flying off with a mere glancing blow of the blade), although there is no blood. Can't say I really gravitate towards gorey games...
 
From what I played I'd have t say Quake III. When you blow someone up stuff would go flying everywhere with lots of blood if I remember right.
 
Shooting someone point-blank with a shotgun in FEAR and having them explode into a huge bloody cloud around you is pretty gory. So is the Type 7 Particle Weapon in that game. 🙂 That thing is a ton of fun.

Consuming hearts of your victims for health, settings them on fire with a flare gun. Blood 2 even had the Phantasm sliver sphere in its expansion pack. Good times 🙂

+1 for Rise of the Triad, that game was rough!

Also add one for Diablo 1 when you get to hell and there are nude bodies that have been skewered and displayed all over the place. I was glad to see them gone in Diablo 2.

what you gained in relief you lost in atmosphere 😉

as creepy as it was, that's what made diablo1's atmosphere superior, IMO 🙂

sometimes while playing... I could almost smell the burning flesh and feel the blood spilling on me... wanted to take a bath after each game...
 
Myth2 for pure gore factor. Bodies do not dissapear and satchel charges lead to mass explosions of entire armies, leaving body parts strewn all over the place like the scene of a mass execution grave.
 
soldier of fortune 1 and 2; rise of the triad +2; postal 2 (pee on the dead bodies of victims); kingpin: life of crime; Severance, Blade of Darkness (hack enemies to bits); Gears of War (chainsaw anyone?)
 
Wow, all expect for Doom and Quake, I have not played any of them. I think I'll check into blood and Postal 2. Sounds like a crap load of fun.
 
postal 2? but i guess its violence is sort of cartoonish...mortal combat cartoonish. can't think of anything heinously violent.

I guess you could consider setting people on fire and throwing diseased cow heads to sicken the general public "cartoony."
