There's a Blackout restrictions to watching local team games.

Say if you ask your cousin from California to sign up and let me use the account, would it work or does mlb.tv track the IP address of the region you are in?

It uses your IP address. I know this cause I bought MLB.tv for a month. You can use anonymous proxy servers to get around it however.
 
I think they track it by the credit card you sign up with and the billing address zip code. When I went to log on for the first time it asked for my cc info for address verification purposes for blackouts
