Plenty of people get medical care and ask to be billed for it, this is what people who self insure do. This is essentially what insurance is, they perform the service and then bill the insurance instead of you.

the only difference being that insurance companies have statutory requirements to carry enough capital to cover their losses, including tail events.

You have no such requirements and would certainly get wiped out, while the hospital has no choice but to perform the service.
