This happened to me when I was about 16 or so. I also had insurance and thought it was no big deal so I forgot all about the court date.
Nothing is ever certain in legal matters but there is a 99.9% chance a warrant for your arrest was issued. (it was in my case and it "always" is when you fail to appear for court. It's the court's only way of making you appear, they don't just drop the matter)
If so, there is an very good chance that you will be arrested if you show up at the courthouse and tell them who you are. In my case I was advised by a friend to call the clerk and inquire about my status. It turned out the bond was already set and that while I would still be arrested, all that would happen was that I would be arrested, paperwork filed, bond paid, and released. In and out in an hour or so as long as I brought the money to pay the bond. (~$370.00 in my case)
So, call and see what you can find out before you show up. You have nothing to lose.
