Note: Refer to the warranty or call the toll free number if you have any questions about Microsoft's policy. Below I'll outline the information given to me over the phone by a Microsoft Supplemental Parts rep.

If you call Microsoft Supplemental Parts (800-360-7561 toll free USA & Canada) they will give you a fax (716-447-7330). All you have to do is fax them the following:

1) To: Microsoft Supplemental Parts (<- rep said this was important)
2) your first & last name
3) address
4) telephone number
5) email address
6) photocopy of the bottom of your mouse, with a setting clear and light enough to show the Product ID information on the mouse.

I tried calling Microsoft to verify that all I needed was to send the above information, but the guy said that he needed to know how long ago I purchased the mouse. He also said he needed a receipt. My mouse needs to be replaced because the buttons aren't working as well as they once did (it's several years old) and I was hoping I could get a new one for free. Has anybody else recently called Microsoft and asked about this policy?

Why did you call? Rule no.1: Never Ever call. Do what the instructions tell you to do, and you should be fine. If you don't beleive me, check out this 12 page thread:

Well, I didn't give him any critical info. I'm careful not to do that. Anyway, thanks for that link. I still have to read through it.

I used this program a few years ago, I actually had an Intellemouse Explorer on the fritz. They sent me the replacement and I've used it ever since.

Mine which was the 2000 laptop model that broke three months ago,
I then called M$ after a couple of questions they sent me a replacement a 3000 model which now just broke.
Should I call M$ again and tell them you make crappy product or should I just forget M$ and get a LT mouse instead?
