I think you don't understand consumer preference and McDonald's business model. McDonald's does not sell enough salads to make any dent in there total revenue (<1% of sales in salads). The majority of consumers who go to McDonald's do not look to purchase a salad and that's why McDonald's eliminated 4-5 salad choices to 1. Mcdonalds business model is too sell Hamburgers at various price points. The issue now is consumer perception of the product is that its low quality, high priced (compared to competitors), and unhealthy.
The American consumer is not willing to pay $5+ for a premium hamburger from McDonald's because the perception from the buying public is there not getting a good value for the price there paying for the final product compared to a there competitor.
