Making Forms with MS Word

I have two questions for those that are very good at making forms using MS word.

First: How do I make the visible field show larger than 4 characters? When you put in a text entry field, it defaults to a grayed square box 4 characters long, and it looks like crap. Ideally, I'd like to make the field to click on larger so I can remove the grey fields (I leave them there so people know where to click on to enter the information).

Second: I use the forms as Templates. I want the default save option to save it as the name they enter into the "EmpName" field + the name of the form, such as if Bob Smith were filling out an employee evaluation form, the default save name would end up being "Bob Smith - Employee Eval.doc".

Forms are already made, just trying to make it so when employees go to save the forms, it saves it as <employee name> - <form name>.doc
