Making Forms In Word 97?

Is it possible to make a form that can be emailed to people who can then fill out the form? I tried it but each entry of data moves the form text in subsequent lines. Basically, the formatting on the form needs to be "locked" so that it cant' be edited or moved.
Protecting it or saving it as Read Only still doesn't preserve the formatting.
 
Go to View, Toolbars, then show the Forms toolbar.

Place fields where you want people to edit, and then you can lock the form so that they cannot edit non-field areas.
 
I didn't have any luck. I am not sure if I was entering the fields correctly. After doing so, the text still moved when adding the needed info. If I locked it, I couldn't enter anything. Here is a part of the form:

Location____________________________________Date_______________________________________

Supervisor__________________________________Inspector____________________________________

I want them to add the appropriate info, in Word, and then be able to email it.
I should add that I started with a complete word document and I want to convert that to a form.
