What he said. I DO use Ghost often to back up the system at different install times so that if something goes wrong at the next step I can recover, but even I know it's kind of anal.

For a freshening (format) of the OS I go about every 6 months or so (also fools the AV into thinking it's installed new and gives me another year).
 
I only reformat on motherboard upgrade,my XP pro is happy with SystemSuite v5,I run registry cleaner and cleanup every few weeks from within SS5,other maintenance software I use is anti-spyware ie..SpywareBlaster,Spybot,Ad-Aware SE,HiJackThis.

FYI my XP OS is very solid ,no BSOD or crashes and last reformat was nearly 2 years ago.

I forgot to say I also run Diskeeper 8 Pro(set to run in screen saver mode)for regular defrag.
 
Check inside your case and inspect your fans, including your heatsink and heatsink fan, for dust and lint build-up occasionally. Don't forget your hardware cooling efficiency. Wipe the dust+lint build-up from your fan blades with Windex at least every six months. A paint brush is handy for dusting - and I use a vacuum cleaner to remove loose dust.

Check updates for all security programs frequently.

I clean history, clean registry, clean junk files, thoroughly scan, defrag and image my OS partition weekly. Backup or image critical data at whatever frequency suits you.
