Looking for SPDIF-out (w/rca) / Game(midi) combo bracket

As the title says....

I'm looking for a SPDIF-out w/rca out -- and -- a Game/MIDI port combination bracket for a mATX motherboard/case.
Slots are extremely limited, so I _really_ want to find a combo and not have to use 2 slots.

Please post if you know of one
