Looking for Free Knowledge Base Software

Just looking for some suggestions on knowledge base software, preferably free, and locally installable would be even better. Google isn't turning up much that looks promising right now. Just something where I can input "this is what I did when this happened"
Desk.com looks interesting, but its paid and web based (and more for support overall).

I use MoWeS on a thumb drive. It's a very portable wiki.
