Locking up with new v3 2000

I just bought a voodoo3 200 and put it in my amd k6-2+ system. Ever since then, I have been having the computer completely lock up on me, forcing me to reboot. Ive tried a number of different things, but after a few minutes, the system continues to freeze.

Specs:
AMD k6-2+ @450 (75mhz*6)
Asus tx97-e
128 mb edo ram
generic sound card
plextor 12/10/32

Does anyone have any suggestions or seen this before? I dont want to take the card back, but if I cant fix the problem, I will have to.

a lot of v3 2ks, had the glue on the heatsink on too think or not applied fully. I had this happen, just ripped the heatsink off, scrapped off the glue and used superglue and heatsink compound mixed to gether to put it back on

Im pretty sure it wasnt heat, since I stuck a fan on it. That really cooled the thing down, but still it locked up. Voltage might be possible, but I didnt want the hassle with it. Since I might be building a new system soon enough, I just took the card back. Thanks all.
