Friends were thinking of trying out L4D but there are only three of us. I know you can go into a lobby to set up a game and set it to only allow friends but I don't know if that setting carries forward. In other words once we start a game with only three people will any random person be able to jump into that fourth slot?

If you start the game set to "friends only", it remains that way.

Once all your friends have joined, have the lobby leader change it "Public Lobby", & then anyone can join.
 
if you set it as friend only. only person that will be able to join your game will be the person from your friendlist (or your friends' friendlist)
