The cord is supposed to loop above the ear then down your back. The plastic sleeve at the cord split can be pulled up to snug the two ear cords to your head.

Yeah, that's what I have now except with the wire running down my back, it's running down my front. I don't like it when it runs down my back.

Try it for a few days. It's a bit awkward at first, but once you get used to it, it's a much more secure and comfortable fit. The cord in the front tends to tug downward on your ears (feels like the buds want to pop out). When you loop around the ear, the buds stay in place with no tugging.

I had the best luck initially with the small foam plugs. I recently bought the soft flex plugs from the Shure website, and I think I like them the best.
 
personally mine were very uncomfortable for awhile. I couldn't listen for extended periods. It wasn't the fight of the earphones persay : it was the pressure in my head i guess. I've gotten used to them now and love em. Keep wearing em, the pain might be around for a bit till you get used, but it shouldn't be crazy intense.
 
Yeah, but with the weight of all the wire torqueing your inner ear I bet that's a part of why it's uncomfortable.
 
Run it behind your ear, but you don't need to run it behind your neck. It will prevent the earbuds from sliding out of your ear.
