If I'm thinking correctly, (that might not happen all that often) when someone is a leper, parts of them may fall off. So. . .it's a play on words. The leper's "tip" must have come off during sex, but it could have also been interpreted as a tip that you'd leave someone like a waitress. Does that make sense?
