Damn... I'm glad I load my own ammo.
As a matter of fact one time about ~10 years ago my dad was making some ammunition and he miscalculated how much powder to put in. (Don't remember what kind of cartridge it was.) Long story short the gun exploded but luckily he was not seriously injured.

I've experienced bad ammo. One time we were plinking out in the woods across a small creek. I fired and the bullet fell into the creek about 5 yards away. Must have been primer only and no powder. It was a definite WTF moment.
I thought about blocked bores more often after that.
