- If you have dreams of doing this, we should probably not go any further.

I've always just said "tuna." I think the people who first did it were trying to differentiate between the distinctly different smell of tuna and the fishy smell that other fish make when saying what something else smelled like.

I think it's because most other "fish sandwiches" are typically breaded and fried/baked fillets while a tuna fish sandwich is usually shredded tuna mixed with mayonnaise (basically a tuna salad sandwich). Usually tuna served in other ways like steaks or melts usually aren't called "tuna fish" just "tuna", so my guess is that "tuna fish" came into use just to distinguish it from other fish sandwiches, and due to the popularity of tuna fish sandwiches.

But how would "tuna" alone not be enough to distinguish in that setting?

Tuna was the name of prickly pear cacti for hundreds of years before being used for the fish.

Litterally the only human being I have heard say "tuna fish" is the guy in the matrix.
