I think this really depends on the area you live in. Around here it is rare to see a pay first before pumping type gas station. I often pay cash, and they activate the pump, I fill up, and then walk inside to pay.

But she is in the middle of pumping! How could they know they intended to not pay for the gas?

Because the car was running, which is likely why he had a fire extinguisher too (you are supposed to turn it off when you pump).

Yes, that's the whole idea... 1) not to rip the pump out of the ground, and 2) to shut off the flow of fuel. There is a replaceable 'key' that is designed to shear off under load, and then a ball valve seats to stop the flow. Maybe the key didn't give way and it ripped the hose out of the bottom of the breakaway...
