It's not that. There's a real mental thing called "bladder shyness" that makes it impossible for some people to urinate in front of others. They aren't shy about exposing themselves which is why it isn't called "cock shy" or something else.

It isn't depicted in media often so few people are aware of it. I know there was a scene in "Real Genius," but the kid was acting reclusive for other reasons in that scene so I didn't catch it when I first watched it.
