Why not just write :
Moved. Forward to:

123 Main Street
Anytown USA?

The post office will deliver it to your new address without having any extra postage attached.
(at least for 1st class mail...packages not included)

You can mail just about anything, regardless of how it's packaged, and the post office will attempt to deliver it.
