is there anything like cool n quiet for GPUs?

Yes, my X1900XT downclocks itsself when idle. Well actually i dont know what it does, but it uses significantly less power when idle than under load, i assume it does downclock itsself. The fans also spin down. Not really like C&Q since it cant really be enabled or disabled but its similar.
 
I don't think there's software to scale GPU clock rate based on load like Cool n Quiet. The driver already downclocks when in 2D mode and switches back to full speed in 3D mode. I suppose you could fiddle with the clock rate and run your 2D mode a little lower, but I'm not sure what that would accomplish.

EDIT: did you check your 2D clock rate in your driver?

I'd imagine you could set up something like RivaTuner to do this for you - set core/mem clockspeeds and voltages to something low but stable at startup, and then use 3D app detection to switch to a full load profile when needed, then back to low speed for the desktop.

you could always go with a fanless GPU, though i don't think that would work for something on the order of an X1900/7900. they have fanless 7600GT's, don't they? (GT, not GS)
 
If you are using an ATI card use can use ATI tools to set different profiles. That is what I do. I have it set to load up downclocked and I change the profile to the normal or overclocked when I start a game.
