Just the angle of the shot, and the fact that there's a very bright light source on his left side and some light from his above-right, so there's deep shadows. On the right side of his neck, it's in shadow because of his jaw, but somewhat lightened by reflected light. On his left below the collar it's very dark because of the extra bright light on his left, and no light coming downward. They way the lights are set up just causes sharp delineations.

And some people do just have big heads.

It's a wide angle lens, used badly.

The lighting is the same from the face down to the neck; 1.5 stops over.
