Is incest illegal in all forms?

Or is it just illegal with minors? What is defined as incest? Cousins? When did it become illegal and why, sure it's abnormal and isn't advantageous for any sane purposes but why make it illegal? Anyways, please answer the first question, the rest aren't really important.
