Is 4.86 on the +5.0v rail bad?

I think it is.

I have:
1ghz Tbird
5 80mm Case fans, 4 of them have Leds
2 CD-Rom drives
2 7200rpm HDDS
Radeon8500
Sound card
a front I/O port a dapter, I'm sure this pulls power some way or another
2 256MB PC-133
a blue CC that flickers when I turn it on
nic card
usb mouse
PS/2 Keyboard
headphones

all sitting pretty on a 300w Antec PSU. I think its time for an upgrade, I was thinking at least 400w.

I think that is just fine actually and you don't really need a new power supply. Higher wattage power supplies are just a marketing technique that works well. Any 300W AMD approved power supply will power just about any desktop out there without a sweat.

That is true, it is a bit of a marketing campaign, but when you are wondering why you cpu wont clock, you can look at your 5v rail at 4.94 (while running 12 case fans) and know that the ps cant be the problem. :)

The AIW 9700 actually will be shipping with a minimum requirement of a 300 Watt PSU. Depending on other things on your system, I think we're moving into the 400 and up realm from here on out.

Uh, fans don't run off of the 5v rail, it's the 12v rail....

Very few fans run on 5 V (only the tiny fans used in laptop PCs for instance).

Certainly the 3 pin molex connectors on motherboards supply 12 V for fans.

Is 4.86 V on the +5V supply bad? How are you measuring it? If you are relying on the motherboard monitoring circuits, then that is absolutely fine.

Mine's hovering between 4.7 and 4.84; doesn't seem to have any problems. I haven't had a system crash that required a reboot for several days now. The only programs that crash are CounterStrike and sometimes Internet Explorer. The program crashes, I close it, and open it again and it works fine; using WinXP Pro.
