Lockout/Tagout.

Always. No exceptions.

Machines don't love you.
They want to hurt you... they need to hurt you.

Doesn't sound like a lockout issue. Sounds like the machine was in normal operation but the moving parts weren't guarded.

A few years ago, a guy in connecticut fell into a vat of heated tar. I guess it was technical drowning. But that sucks.

Also, a guy where I work died when his head got caught in a hydraulically operated door.

Welcome to the internet. We may seem insensitive with our jokes, but we all are terribly sorry for your loss and I hope your sister and their kids will be able to move on in sufficient time.

Now that our fun has been brought to a grinding halt by a relative, on to another unfortunate situation to make inappropriate puns.

This. You lockout tagout when a machine is down and usually for maintenance or repairs. It's so some jackass doesn't come by and flip the machine on while you're working on it.

This is the type of machine in question.

That's a milling machine. I'm not sure that's what he fell in. The story made it sound like he was at a plant that crushed old blacktop. I did an image search when this thread popped up, and saw some likely candidates, but I'm not familiar with that aspect of construction.

Happened on the highway here a couple years ago. Truck rolled and spilled a lot of scalding hot tar all over the driver. Guy survived though. Was rescured by a bystander.

Rolando Anaya got caught in a conveyor belt of a mobile asphalt crusher about 12:06 p.m. Monday, Oct. 7, along the 25200 block of Maitri Road, according to the sheriff’s coroner and Cal Fire officials. He died at 12:24 p.m.

Worst 18 minutes ever.

My bad. I just assumed it was one of those machines they use to tear up the roadway prior to laying new asphalt. The article just said mobile asphalt crusher so I assumed that this was what took his life.

Horrible way to go... :'(
