I/O Disk Error Whats the cause?

Its coming from a floppy drive. The cable is plugged in right and the mobo does see the floppy drive so is it just a bad drive?

i/o 40 error is either:
bad floppy
bad floppy cable
floppy cable on backwards on the mobo or drive
bad floppy connector on mobo

have an extra 1.44 lying around?
