get rid of anything Yamaha - SYG-50/75 midi synthesizer, yamaha chip based soundcard, SAX -3 sound
(default microsoft) drivers
their drivers conflict with nvidias screwing up rendering of icons

Run 16 bit mode..and not 32 bit mode..
for some reason my Geforce didnt like to do 32 bit color.. for a long time i was running 16 bit. then the other day I plugged it into a another monitor (and had to change to VGA 256) and then went straight to 32 bit and since then its worked.. even on my old monitor.
dont ask me what happened.. but if all else fails.. run 16 bit.

Your icon cache might be corrupt. TweakUI has an option to rebuild desktop icons.
