How to introduce one's self to a girl

Okay, I don't know how to do this so I figured I would go to the place that would give me the most options. How do I go up to a girl I find attractive, but who doesn't know me and I don't know her and introduce myself?

If you went to school with her, it's easy- sit near her at class one day, then turn to her and start talking about the prof, book, homework, etc.

Bullshit. I havent had any problems using the Hi my name is strategy.

I see her in the cafeteria, she lives in a dorm near me...

I did that once.. there was this one girl that I'd see *ALL* the time, at the cafeteria, outside my dorm, walking from class, etc. etc. etc., so one day I was kind of free and she was by herself and I just sat down next to her and was like "I see you ALL the time around here, but I've never met you before, so I've always thought it was odd"

We hit up a conversation, traded AIM names, and now chat every once in a while still (chatted a LOT last year, but kinda died down). Thing is though was that I did see her EVERYWHERE - her classes were like right before or after mines in the same lecture hall, we'd eat at the same time, and she would only get cell phone reception like right outside my dorm window so I'd see her often there too. Because of that, it wasn't too hard to get to know her.
